Passed away at Queens Hospital on Monday 21st April 2014 aged 76 years.
Much loved dad of Mark, Matthew and Mandy,
Dale will be sadly missed by all family and friends that knew and loved him.
The funeral service will be held in the Carnarvon Chapel, Bretby Crematorium on Friday 9th May at 9.45am.
Family flowers only please. Donations in memory of Dale may be placed in the collection box at the service and will be forwarded to Macmillan Cancer Support.
