In loving memory of John Leonard Slack who Passed away peacefully in the Nightingale Macmillan Unit, on 21st September 2015 aged 63 years.
A much loved husband of Maureen (Mo).
A loving dad to Chris and Hannah and Chris's Partner Kim. An adored granddad to Madeleine.
John was a much loved brother to Rose and Sue, and brother in law, uncle and a good friend to many.
He will be greatly missed by all of his family and friends.
The family have requested that something grey is to be worn in support of Brain Tumour awareness.
